Please call the office to make an appointment or request an appointment online. If you are unable to keep your appointment, please call us in advance so that we may reschedule your appointment and use that time to see other patients in need of care.  We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to medical emergencies or surgery.

How to Prepare for Your Appointment


We encourage all our patients to utilize our online patient portal to complete registration and upload pertinent medical history. Otherwise, please plan on arriving early to complete your registration and paperwork. You can find the appropriate forms in the Patient Forms section below.

Please bring the following items to the appointment:

  • Insurance card(s)
  • Referral forms (if required by your insurance plan)
  • List of all prescription and over-the-counter medications that you are currently taking
  • A form of payment (credit card, debit card, check, or cash)
  • Pertinent medical and surgical history information
  • X-ray films or other prior imaging that you may have

Patient Forms


We prefer that all our patients complete their forms using our patient portal via Exscribe.

  • For new patients, a separate link with a username will be emailed to you when you schedule your appointment.
  • For existing patients, please login with your username and password using the link sent to you via email or the link provided: National Sports Medicine Patient Portal

If you have lost your username and password, please contact our office at (703) 729-5010 and we will provide you with your username.

Prescription Refill


Please call the office during regular office hours to request prescription refills. This will allow the physician time to review your chart and make the best decision regarding your care. When calling for a prescription refill, please have your name, date of birth, daytime telephone number, the name of the medication/dosage and the name and phone number of your pharmacy.

Your pharmacist may fax your refill request to our office at (703) 729-5833.

We do not provide prescriptions on weekends or after regular office hours.

Medication Disposal


If you have unused or expired medications to dispose of, there are multiple drug collection units available, providing patients with a safe and environmentally responsible way to dispose of unwanted, unused, or expired medication, including controlled substances.
